The Program provides a group term life insurance and accidental death and dismemberment policy in the amount of $10,000.

Spouse coverage is available up-to $2,000 and up-to $1,000 for each child.

Participation Fees:

• A $25 enrollment fee (covers all elected officials)
• Individual rate is capped at $6.40 per month (subject to change)
• Family rate of $7.40 per month (subject to change)
